Partnerships between health and community service providers are critical for affecting the change necessary to improve the health and well being of the population.
The Connecting Healthcare in Communities initiative in conjunction with the Gold Coast Primary Care Partnership Council welcomed individuals from across the wider health and community sector to join us on the Gold Coast to hear evidence of successful community partnerships and learn skills from those with years of experience at the partnering coal face.
